These are special forms used for cases where review takes place in multiple departments. They take the place of forms with equivalent, non-"Multi-department Review" names. For example, a multi-department review case would use the Certification Form #2: Multi-department Review form instead of the regular Certification Form #2.


These forms feature additional fields where the certification of each department's materials can be independently logged and dated. They are also notable in that they are submitted more than once. When a candidate is ready to certify the first department's materials, they fill out the first set of fields and submit the form. Then when they're ready to certify materials from the next department, a case administrator unlocks the form to allow the candidate to fill out the next set of fields (but they do not change the information in the first set), and submit the form again.
