Department staff can add Certification Forms to Interfolio Packets for candidates to review. Different types of cases can require different forms. When a form is required, the staff will add it to the packet as a candidate requirement and the candidate will review it and sign off on it in Interfolio.
The following Forms are available in Interfolio. Department staff are responsible for adding them if and when they're required for a particular case.
- Candidate Certification: Candidate Documents - this form should be added when the Candidate packet has been assembled, prior to any reviewing group seeing the packet. By signing this form, the candidate indicates that they have reviewed the Data Summary, Self Statement, Teaching Evaluations, etc. They've had the opportunity to ask questions about this review and add materials to it.
- Candidate Certification: External/Internal Letters - if the case has Evaluation Letters, this form should be added after they have been added to a packet, but before the department or any committee begins to review the packet. By signing this form, the candidate indicates that they have reviewed the redacted letters and had an opportunity to submit a response.
- Candidate Certification: Ad Hoc Committee Report - if the case has an Ad Hoc Committee, this form should be added after their report has been added to the packet. By signing this form, the candidate indicates that they have reviewed the redacted ad hoc report and had an opportunity to submit a response.
- Candidate Certification: Standing Committee Report - if the case is reviewed by a Standing Committee, this form should be added after their report has been added to the packet. By signing this form, the candidate indicates that they have reviewed the report and had an opportunity to submit a response.
- Candidate Certification: Department Vote/Report- this form should be added after the department has made a recommendation on the case. By signing this form, the candidate indicates that they have reviewed the Vote page and Chair's report and had an opportunity to submit a response. Once a candidate submits this form, the department staff will forward the case to their Dean's Office.
There's a second copy for the Certifications for reports--for example "Candidate Certification: Standing Committee Report (1)" and "Candidate Certification: Standing Committee Report (2)." The second, "(2)," copy is for situations where a second report is added that needs to be reviewed and certified by the candidate after they've already completed the "Candidate Certification: Standing Committee Report (1)" form.
