An Interfolio packet must include a Cover Page.  Sometimes the Case Summary from Opus can serve as the Cover Page, other times the Cover Page template from the APO website (the “traditional Cover Page”) must be used.

 

First, determine if the Opus Case Summary can be used as the Cover Page.

The Opus Case Summary may not be used as the Cover Page when:

  • the case is going to CAP;
  • the candidate has multiple actions at once.

If there are relevant details for the case that aren’t displayed on the Opus Case Summary, you have to use the traditional Cover Page to add whatever information the reviewers need to know.

 

If you are using the traditional Cover Page, download the correct Data Summary template at https://www.apo.ucla.edu/policies-forms/forms/datasummary/data-summary-forms.  There are several different Data Summary templates, depending on the action and the candidate’s series and rank.  You must extract the Cover Page from the rest of the document so that the Cover Page you fill out and upload to the Interfolio packet is a single page.  The Cover Page should be the first file in the Data Summary section of the packet.  

 

If you use the Opus Case Summary, click the View Appointment Details link to expand the page and show the candidate’s current appointment(s):

 

 

Now that the current appointment(s) section is expanded, you will see a suitcase icon by an appointment to signify that this action will affect that appointment.  In the screenshot below, it’s a Merit that will advance the candidate a step in both departments (Geography and Psychology) if approved.   Print this page out as a PDF and upload as the first file in the Data Summary section of the Interfolio packet.
